Why does Therapy take so long?

One of the most common questions people ask when starting therapy is:

"How long will this take? I know I have to do something different and but why can’t I do it?"

Most people understand what they should do. They know they should stop procrastinating, stop avoiding difficult conversations, stop self-criticizing, stop using unhealthy coping strategies, or start setting boundaries. Yet, knowing what to do and consistently doing it are often two very different things.

The answer lies in understanding how behavior is learned, how the brain changes over time, and why lasting change requires repeated practice rather than insight alone.

The Behavioral Perspective: How Patterns Develop

From a Dialectical Behavior Therapy (DBT) perspective, problematic behaviors are not signs of weakness, laziness, or lack of motivation. Instead, they are learned responses that developed for understandable reasons. Most behaviors begin because they solve a problem in the short term.

For example:

· Avoiding a stressful situation reduces anxiety.

· Isolating decreases emotional demands.

· Self-criticism creates a sense of control.

· Anger may temporarily help someone feel powerful or understood.

When a behavior produces a rewarding outcome or relieves discomfort, the brain is more likely to repeat it in the future. Over time, a simple sequence develops:

Situation → Behavior → Consequence

If the consequence is rewarding or relieving, the behavior becomes reinforced and increasingly likely to occur again. Many of the behaviors clients seek help for today were once effective short-term solutions. The problem is that solutions that work in the moment often create larger problems over time or stop being effective.

How Behaviors Become Habits

Human behavior is learned through several mechanisms:

Direct Reinforcement

We repeat behaviors that lead to rewards or relief. For example:

· A child receives praise for cleaning their room.

· A teenager receives entertainment and social connection from checking their phone.

· An adult avoids a stressful meeting and experiences immediate relief.

In each case, the consequence strengthens the likelihood of repeating the behavior.

Observation and Modeling

People learn by watching others. For example:

· Children observe how parents handle emotions.

· Adolescents learn from peers.

· Adults continue to model behaviors they see rewarded in their environment.

Rules and Instruction

Many behaviors are learned through direct teaching and instruction. However, simply knowing a rule rarely guarantees behavior change.

Repetition

Perhaps most importantly, behavior becomes automatic through repetition. Just as driving a car or playing an instrument becomes easier through practice, emotional and behavioral responses become stronger the more often they are used.

What Happens in the Brain During Learning?

Every time we engage in a behavior, networks of neurons fire together. Neuroscientists often summarize this process with a simple phrase:

"Neurons that fire together, wire together."

Repeated activation strengthens connections between neurons through a process called synaptic plasticity. Imagine walking across a field. The first time, you leave only a faint trail. After hundreds of trips, a visible path develops. After thousands of repetitions, a well-worn road appears. The brain develops pathways in much the same way.

Repeated behaviors strengthen:

· Neural connections

· Communication efficiency between neurons

· Speed of activation

· Automaticity

As pathways strengthen, behaviors require less conscious effort and become increasingly automatic.

The Role of Reinforcement in the Brain

When a behavior leads to something desirable, the brain's reward system becomes activated. Brain regions involved in learning and reward release dopamine, a neurotransmitter that functions less as a pleasure chemical and more as a learning signal.

The brain essentially records:

"That behavior worked. Remember it."

The same process occurs when behaviors help us escape or avoid distress. When anxiety decreases after avoidance, or emotional pain decreases after withdrawing from others, the brain receives another powerful message:

"That solved the problem. Use that strategy again."

Because relief is such a powerful reinforcer, behaviors that reduce emotional pain often become deeply ingrained, even when they create significant long-term consequences.

Why Insight Alone Is Not Enough

Many clients become frustrated because they understand what they need to do. They know the skills. They know their patterns. They can explain exactly why a behavior is unhelpful. Yet they continue doing it.

This occurs because understanding a behavior and replacing a behavior are two separate processes. Insight changes awareness. Practice changes the brain.

A person may understand that approaching anxiety is healthier than avoiding it. However, if they have spent ten years avoiding anxiety-producing situations, the avoidance pathway has been reinforced thousands of times. The new pathway has only been practiced a handful of times. The brain naturally defaults to its strongest pathway.

Why Old Behaviors Feel Automatic

As behaviors are repeated, they become increasingly efficient and automatic. Eventually, the brain no longer pauses to evaluate options.

Instead, it responds with familiar patterns. This is why people often say:

· "I wasn't thinking."

· "I went on autopilot."

· "It just happened."

The behavior has become neurologically efficient. In many ways, therapy is helping people build a new road while simultaneously choosing not to travel the old one.

Why Change Is Even Harder During Stress

When emotions become intense, the brain shifts resources toward survival and threat detection. Under stress, people become more likely to rely on old habits and less likely to access newly learned skills.

This explains why many clients report:

"I know the skill when I'm calm, but I can't use it when I'm upset."

The problem is not a lack of knowledge. The problem is that the new skill pathway has not yet been strengthened enough to compete with older, more established patterns under stress.

Why Therapy Takes Time

Behavior change requires three things:

1. Reducing reinforcement for ineffective behaviors.

2. Increasing reinforcement for effective behaviors.

3. Practicing new behaviors repeatedly across many situations.

This process cannot be rushed. The goal is not simply to understand new behaviors. The goal is to strengthen them until they become the brain's preferred response.

Just as someone cannot become a skilled pianist by reading about piano, emotional and behavioral change requires repeated practice. Insight opens the door. Practice creates lasting change.
